In order to complete registration, you will need to bring:
•Your insurance card
•Your driver's license
In addition, we ask you bring medical records, laboratory reports, and any prescriptions or over the counter medications you are taking. The name of your pharmacy and the phone number is required to send your prescriptions electronically.

If you don't have access to a printer, please plan to visit at least 15 minutes early for your first appointment.
